Eating disorders (ED) and affective disorders (AD) in adolescent population and several investigations have pointed out that specific family dynamics play a major role in the onset, course, and maintenance of both disorders. The aim of this study was to extend the literature of this topic by exploring differences between parents' personality traits, coping strategies, and expressed emotion comparing groups of adolescents with different mental conditions (anorexia nervosa vs. affective disorder vs. control group) with a case-control study design. A total of 50 mothers and 50 fathers of 50 girls with anorexia nervosa (AN), 40 mothers and 40 fathers of 40 girls with affective disorder (AD), and 50 mothers and 50 fathers of 50 girls with no pathology that conformed the control group (CG) were measured with the Temperament and Character Inventory (TCI), the COPE Inventory, the Family Questionnaire (FQ), and psychopathology variables, anxiety, and depression. Both parents of girls with AN showed a significant difference in personality, coping strategies, and expressed emotion compared to both parents in the CG, while they presented more similarities to parents of girls in the AD group. Identifying personality traits, expressed emotion, coping strategies, and psychopathology of parents and their daughters will allow improvements in the interventions with the adolescents, parents, and families.

Las enfermedades dermatológicas que cursan con un patrón reticular son múltiples y variadas. Aunque dicho patrón particular de presentación morfológica muchas veces es muy distintivo, usualmente es poco discutido y estudiado en el contexto clínico. A menudo, estos patrones no se abordan como una categoría diagnóstica propia. Asimismo, las etiologías de este grupo de enfermedades son diversas, desde causas vasculares, infecciosas, tumorales, inflamatorias, metabólicas o genéticas. Además, pueden variar desde condiciones relativamente benignas hasta enfermedades graves que amenazan la vida. Este artículo tiene como objetivo discutir la enfermedad de la piel que se manifiesta con lesiones reticulares y se propone un algoritmo de diagnóstico clínico, basado en el color predominante de las lesiones y los principales hallazgos clínicos, para un abordaje práctico inicial (AU)

A fundamental understanding of the acid gas (H2S and CO2) chemistry is key to efficiently implement the desulphurisation process and even the production of clean fuels such as hydrogen or syngas. In this work, we developed a new kinetic model for the pyrolysis and oxidation of hydrogen sulphide by merging two previously reported models with the goal of covering a wider range of conditions and including the effect of carbon dioxide. The resulting model, which consists of 75 species and 514 reactions, was used to conduct rate of production and sensitivity analysis in plug flow reactor simulations, and the results were used to determine the most prominent reactions in which hydrogen sulphide, molecular hydrogen, and sulphur monoxide are involved. The resulting list of important reactions was screened and the kinetics of three of them, i.e., SO2 + S2 → S2O + SO, S2O + S2 → S3 + SO, and SO + SH → S2 + OH, was found to warrant further investigation. With the goal of improving the accurancy of our new kinetic model, we carried out a robust quantum chemistry and Rice-Ramsperger-Kassel-Marcus master equation study to obtain, for the first time, the forward and reverse rate constants for those three reactions at temperatures and pressures of interest for combustion and atmospheric chemistry. This work is the first step of a kinetic study that is aimed at improving the understanding of the chemistry of the pyrolysis and oxidation of H2S, highlighting the importance of sulphur-sulphur interactions and providing a fundamental basis for future kinetic models of H2S not only in the field of combustion, but also in atmospheric chemistry.

Disorders associated with body dissatisfaction such eating disorders (ED) and muscle dysmorphia (MD) in males are understudied and surrounded by controversy regarding their nosological aspects. The current study examined the prevalence rates of clinical cases of ED and MD through a two-phase study with gold standard clinical interview in a representative sample of 850 Spanish undergraduate men, of whom 141 were interviewed. Levels of body dissatisfaction, compulsive exercise, anxious-depressive symptoms and the amount of physical activity were also explored. A prevalence rate for ED of 1.4% and 1.3% for MD was found. No differences were observed between the clinical groups on scales related to body image, supporting the current perspective that MD as well as ED and Body Dysmorphic Disorder could be clustered in a new spectrum of body image disorders, where the behaviours performed to achieve body change could range from restriction or muscularity-oriented eating alterations to pathological exercise or cosmetic surgery. The usefulness of the cut-off points of the screening questionnaires in MD and ED in males are discussed.

La traqueotomía percutánea por dilatación es un procedimiento que se realiza en las unidades de paciente crítico, implica la disección roma de los tejidos pretraqueales, seguida de la dilatación de la tráquea sobre la guía y la inserción de la cánula traqueal mediante la técnica de Seldinger. En las últimas décadas, la evidencia sugiere que, en manos de médicos capacitados, es al menos tan segura como la traqueotomía quirúrgica, con similar incidencia de complicaciones. La selección adecuada de pacientes y el uso de herramientas de seguridad complementarias, como broncoscopio o ultrasonido, disminuyen las tasas de falla y complicaciones. Siendo contraindicaciones absolutas para traqueotomía percutánea por dilatación una anatomía anormal, tumor maligno en el sitio de traqueostomía, coagulopatías o vía aérea difícil. La guía mediante broncoscopia permite la evaluación de la profundidad del tubo endotraqueal, confirma la posición de la aguja en el eje de la tráquea y la adecuada inserción del cable guía y dilatador. Entre sus desventajas destacan que, el sitio de punción está sujeto a sesgo y no puede guiar con precisión la aguja en la penetración de la tráquea. La traqueotomía percutánea guiada por ultrasonido es una alternativa validada en unidades, donde no se cuente con broncoscopia. Es un método rápido, seguro, que permite la identificación de estructuras anatómicas, vasculatura cervical, permite identificar el sitio de la punción y guía la inserción de la aguja en la tráquea. Esta técnica presenta altas tasas de éxito al primer intento, reduciendo significativamente el número de punciones.


Percutaneous dilation tracheostomy is a procedure performed in critical patient units. It involves blunt dissection of the pretracheal tissues followed by dilation of the trachea over the guidewire and insertion of the tracheal cannula using the Seldinger technique. In recent decades, evidence suggests that in the hands of trained physicians it is at least as safe as surgical tracheostomy, with a similar incidence of complications. The proper selection of patients and the use of complementary safety tools such as bronchoscope or ultrasound reduce failure rates and complications. Being absolute contraindications for PDT abnormal anatomy, malignant tumor at the tracheostomy site, coagulopathies, or difficult to treat airway. Bronchoscopy guidance allows evaluation of the depth of the endotracheal tube, confirms the position of the needle in the axis of the trachea and the proper insertion of the guide wire and dilator. Among its disadvantages are that the puncture site is subject to slant and cannot accurately guide the needle into the trachea. In addition, it requires Critical Patient Units with bronchoscope and trained personnel. Ultrasound-guided percutaneous tracheotomy is a validated alternative in units where bronchoscopy is not available. It is a fast, safe method that allows the identification of anatomical structures, cervical vasculature, identifies the puncture site and guides the insertion of the needle into the trachea. With high first-attempt success rates, significantly reducing the number of punctures.

Introducción: La sialorrea es la pérdida involuntaria de saliva de la boca, ya sea debido a la producción excesiva de saliva o disminución de la frecuencia de deglución. Se habla de sialorrea patológica cuando persiste más allá de los 4 años de edad. Además de las implicaciones sociales, cambios de ropa frecuentes, puede provocar neumonías por aspiración y deshidratación. El manejo de la sialorrea requiere una evaluación completa con un enfoque de equipo multidisciplinario para el tratamiento, que incluye terapias no farmacológicas, farmacológicas y quirúrgicas. Objetivo: Presentar resultados quirúrgicos y farmacológicos en el tratamiento de sialorrea masiva. Material y Método: Se realizó revisión de historias clínicas de 7 pacientes portadores de sialorrea masiva. Todos los pacientes incluidos fueron refractarios a tratamiento médico. El diagnóstico fue obtenido por un equipo multidisciplinario. Se les realizó desfuncionalización quirúrgica y farmacológica de glándulas salivales. Se les aplicó Escala de Severidad (DSS) y escala de frecuencia (DFS), previo a cirugía y posterior a procedimiento hasta el año. Resultados: Mejoría clínica subjetiva posterior a desfuncionalización quirúrgica con disminución de DSS y DFS. Disminución promedio de baberos a 10/día. Conclusión: Los resultados obtenidos son buenos, si se consideran las escalas DSS, DFS y el número de baberos al día, que son mediciones tanto subjetivas y objetivas respectivamente.


Introduction: Massive Sialorrhea is the involuntary loss of saliva from the mouth, either due to excessive saliva production or decreased swallowing frequency. We speak of pathological sialorrhea when it persists beyond 4 years old. In addition to the social implications and frequent clothing changes. It can cause aspiration pneumonia and dehydration. Treatment for sialorrhea requires a comprehensive evaluation with a multidisciplinary team approach. Including non-pharmacological, pharmacological, and surgical therapies. Aim: Presentation of the results of surgical defunctionalization of the salivary glands plus injection of Botulinum Toxin in the treatment of massive sialorrhea. Material and Method: A review of the clinical records of 7 patients with massive sialorrhea was carried out. All included patients were refractory to medical treatment. The diagnosis was obtained by a multidisciplinary team. Surgical and pharmacological dysfunctionalization of salivary glands was performed. Severity Scale (DSS) and Frequency Scale (DFS) were applied before surgery and after the procedure up to a year. Results: Subjective clinical improvement after surgical defunctionalization with decreased SHD and DFS. Average decrease in bibs to 10/day. Conclusion: The evaluated strategy presented similar benefits with respect to the literature. The SHD and DFS scales and the number of bibs per day are both subjective and objective measurements, respectively, and allow the clinical improvement and quality of life of patients undergoing surgery to be evaluated individually.

El colesteatoma congénito (CC) es una lesión benigna de epitelio escamoso queratinizado que puede afectar diferentes aéreas del hueso temporal con predominio en el oído medio. El CC es una patología poco frecuente que se presenta en pacientes pediátricos y clínicamente se manifiesta como una lesión blanquecina detrás de un tímpano indemne. La mayoría de los pacientes no presenta historia de hipoacusia, otorrea, infección, perforación o cirugía otológica previa. Se analiza el caso de un prescolar con CC que consultó con trastorno de sueño sin sintomatología otológica, pero con otoscopía alterada como hallazgo clínico. En la resonancia magnética nuclear (RMN) con secuencia de difusión, se evidenciaron hallazgos sugerentes de lesión colesteatomatosa en oído medio. Se realizó tratamiento quirúrgico endoscópico combinado con remoción completa de la lesión compatible histológicamente con CC y reconstrucción funcional con prótesis inactiva con resultado auditivo satisfactorio. El CC requiere alta sospecha diagnóstica por pediatras y otorrinolaringólogos, siendo imprescindible realizar un examen físico acucioso que incluya otoscopía de rutina, aunque el paciente no manifieste síntomas otológicos. El tratamiento es quirúrgico y debe considerar uso de endoscópico para asegurar una extracción completa de la lesión. En algunos casos es requerido realizar una reconstrucción auditiva para asegurar un óptimo resultado funcional.


Congenital cholesteatoma (CC) is a benign lesion of keratinized squamous epithelium that can affect different areas of the temporal bone, predominantly in the middle ear. CC is a rare pathology that occurs in pediatric patients and clinically manifests as a white lesion behind an intact eardrum. Most patients do not have a history of hearing loss, otorrhea, infection, perforation, or previous otologic surgery. The following, is the case of an infant with CC who consulted with a sleep disorder without otological symptoms but with altered otoscopy as a clinical finding. Nuclear magnetic resonance (NMR) with diffusion sequence with findings compatible with a cholesteatomatous lesion in the middle ear. Endoscopic surgical treatment was performed combined with complete removal of the lesion histologically compatible with CC and functional reconstruction with inactive prosthesis with satisfactory hearing results. CC requires high diagnostic suspicion by paediatrics and otorhinolaryngologists, and it is essential to perform a thorough physical examination that includes routine otoscopy even if the patient does not show otological symptoms. Treatment is surgical and endoscopic use should be considered to ensure complete removal of the lesion. In some cases, hearing reconstruction is required to ensure optimal functional results.

El tumor de Pott es una entidad rara, definida como un absceso subperióstico asociado a osteomielitis del hueso frontal. Suele presentarse como complicación de sinusitis aguda o crónica del seno frontal, y se describe con mayor incidencia en población pediátrica, siendo una complicación grave por su alta morbimortalidad. Se presenta a un paciente pediátrico con tumor de Pott y absceso cerebral posterior a una sinusitis aguda de foco odontogénico, en la que los pilares de tratamiento son drenaje quirúrgico precoz y antibioticoterapia prolongada. Se describe la clínica, manejo médico-quirúrgico y seguimiento posterior, ya que, en ausencia de antecedentes, se debe buscar dirigida- mente enfermedad periodontal y realizar un estudio de inmunodeficiencia primaria.


Pott's tumor is a rare entity, defined as a subperiosteal abscess associated with osteomyelitis of the frontal bone. It usually presents as a complication of acute or chronic sinusitis of the frontal sinus and it is described with a higher incidence in the pediatric population, being a serious complication due to its high morbidity and mortality. We present a pediatric patient with Pott's tumor and brain abscess after acute sinusitis of odontogenic focus, in which the pillars of treatment are an early surgical drainage and prolonged antibiotic therapy. The clinic, medical-surgical management and subsequent follow-up are described, since in the absence of antecedents, periodontal disease should be sought directly and a study of primary immunodeficiency performed.

Los tumores malignos de cavidad nasal y cavidades paranasales son poco frecuentes y sus síntomas de presentación suelen ser unilaterales e inespecíficos; éstos incluyen: epistaxis, rinorrea y obstrucción nasal. Encontramos una amplia variedad histológica de tumores, entre ellos tenemos el angiosarcoma nasal, que es considerado una rareza y el diagnóstico definitivo lo da la confirmación anatomopatológica complementado con estudios inmunohistoquímicos. Su modalidad de tratamiento incluye múltiples opciones, pero la cirugía radical temprana con márgenes negativos asociado a radioterapia adyuvante, ofrece el mejor pronóstico. Se presenta a un paciente adulto masculino con un angiosarcoma nasal izquierdo que fue detectado de forma precoz y se manejó con cirugía radical y radioterapia.


Malignant tumors of the nasal cavity and paranasal cavities are rare, while presenting symptoms are usually unilateral and nonspecific like, epistaxis, rhinorrhea, and nasal obstruction. There are many histological varieties of tumors, among them we have nasal angiosarcoma, which is considered a rarity pathology. The definitive diagnosis is given by biopsy confirmation complemented with immunohistochemical studies. Its treatment modality includes multiple options, but early radical surgery with negative margins associated with adjuvant radiotherapy offers the best prognosis. We present a male adult patient with a left nasal angiosarcoma that was detected early and managed with radical surgery and radiotherapy.

Los varones son mas vulnerables a realizar conductas patológicas relacionadas con el ejercicio físico. El Cuestionario de Ejercicio Compulsivo (CET) es una de las medidas mas utilizadas sobre ejercicio patológico. Sin embargo, existen algunas inconsistencias con respecto a sus propiedades psicométricas, especialmente en muestras masculinas. El objetivo fue evaluar las propiedades psicométricas de la versión española del CET en una muestra representativa de varones.Un total de 750 estudiantes universitarios varones respondieron al CET junto con el Inventario Revisado de Obsesividad-Compulsividad, Inventario de Trastorno Dismórfico Muscular y Cuestionario Internacional de Actividad Física. Para evaluar la estructura interna, se probaron dos modelos mediante Análisis Factorial Confirmatorio. Se estudió la invarianza de la escala en función del nivel competitivo, la consistencia interna, la validez convergente y concurrente.Los resultados no replicaron la estructura original de cinco factores. Por el contrario, se apoyó una solución de tres factores, así como la invarianza de la escala. La validez convergente y concurrente fue confirmada por la correlación con la sintomatología de dismorfia muscular, la obsesividad y el ejercicio físico vigoroso.La escala muestra buena consistencia interna.La versión española del CET es un instrumento fiable y válido, aunque se requiere de mayor investigación que aborde las cogniciones y conductas patológicas relacionadas con el ejercicio compulsivo, queafectan mayoritariamente a los varones. Men are more vulnerable to engaging in pathological behaviors related to physical exercise. Compulsive Exercise Test (CET) is one of the most used measures of pathological exercise. However, there are some inconsistencies regarding their psychometric properties, especially in male samples. The aim was to psychometrically evaluate the Spanish version of the CET in a representative sample of Spanish males. A total 750 male university students answered to the CET together with the Obsessive-Compulsive Inventory Revised, Muscle Dysmorphic Disorder Inventory and International Physical Activity Questionnaire. To assess the internal structure, two models of the CET were tested by confirmatory factor analysis. Measurement invariance by competitive level, internal consistency, convergent and concurrent validity was also studied. Results did not replicate the original five-factor. In contrast, the three-factor solution was supported, as well as the measurement invariance. Convergent and concurrent validity was confirmed by correlations with muscle dysmorphia symptomatology, obsessiveness, and vigorous physical activity. Adequate levels of internal consistency were shown for the scale. The Spanish CET is a reliable and valid instrument although further research is required addressing specific problematic cognitions and behaviors related to compulsive exercise, which affects mainly males. Os varões são mais vulneráveis a comportamentos patológicos relacionados com o exercício físico. O Teste de Exercício Compulsivo (Compulsive Exercise Test, CET, em Inglês) é uma das medidas de exercício patológico mais amplamente utilizadas. Contudo, existem algumas inconsistências no que diz respeito às suas propriedades psicométricas, especialmente em amostras masculinas. O objetivo era avaliar as propriedades psicométricas da versão espanhola do CET numa amostra representativa de homens. 750 estudantes universitários varões responderam ao CET juntamente com o Inventário Obsessivo-Compulsivo Revisado, Inventário de Desordens Dismórficas Musculares e o Questionário Internacional de Actividade Física. Para avaliar a estrutura interna, dois modelos foram testados por Análise Confirmativa de Fatores. A invariância da escala também foi estudada em função do nível competitivo, da consistência interna, e da validade da convergência e concorrente. Os resultados não reproduziram a estrutura original de cinco fatores. Mas em vez disso, foi mostrada asolução de três fatores, bem como a invariância da escala. A validade convergente e simultânea foi confirmada pela correlação com a sintomatologia da dismorfia muscular, obsessividade e exercício físico vigoroso. A escala mostra uma boa consistência interna. A versão espanhola do CET é um instrumento fiável e válido, embora seja necessária mais investigação para abordar os comportamentos cognitivos e patológicos relacionados com o exercício compulsivo, que afetam sobretudo aos homens. The cadmium L x-ray spectrum induced by electron impact was analyzed in detail. The measurements were performed on a bulk pure sample using a commercial wavelength dispersive spectrometer, and the spectrum was processed with a parameter optimization method previously developed. This procedure permitted the determination of characteristic energies, relative transition probabilities and natural linewidths for this element. The results obtained here were compared to the data found in the literature, when available. Spectral structures related to satellite and radiative Auger Effect emissions were also analyzed, assessing energy shifts and relative intensities. Some of these parameters were determined for the first time, even in overlapping peaks and weak transitions, which was possible due to the robustness of the spectral processing method used.

Resumen Los trastornos somatomorfos (TS) corresponden a un conjunto de entidades de expresión polimorfa cuya característica común es la relevancia de los síntomas somáticos asociados a un malestar psicológico significativo evidente o no, pero sin una base estructural delimitada. Si la sintomatología se asocia al sistema nervioso se denominan trastornos conversivos (TC). Su etiología tiene una naturaleza multicausal y compleja y se expresan en todos los sistemas del organismo. Los TS y los TC han sido poco estudiados en la otorrinolaringología pediátrica y su enseñanza es escasa en la formación médica. Se realizó una búsqueda sistemática sobre TS y TC en otorrinolaringología pediátrica en las bases de datos PubMed/Medline, SciELO y Cochrane Library. Se incluyeron 49 referencias, principalmente estudios observacionales y revisiones narrativas. Los cuadros clínicos descritos fueron el estridor funcional, la sordera psicógena, el trastorno facticio y el vértigo psicógeno. El proceso diagnóstico requiere de la evaluación otorrinolaringológica y psiquiátrica. En todos los estudios se reconoció que los participantes tenían alguna alteración afectiva prominente, aunque a veces oculta. El análisis del contexto social y escolar, además de los antecedentes familiares de TS, TC o cualquier desorden mental son elementos primordiales. La terapia es multidisciplinaria, incluyendo intervenciones otorrinolaringológicas, fonoaudiológicas, psicológicas y psicofarmacológicas. Sin embargo, la evidencia que sustenta a las intervenciones especializadas es escasa. Los niños, niñas y adolescentes con TS y TC presentan hallazgos clínicos y biológicos que no se presentan en los simuladores. Un adecuado diagnóstico y tratamiento se relacionan con un buen pronóstico.


Abstract Somatoform disorders (SD) make up a group of entities with polymorphic expression, characterized by the relevance of somatic symptoms associated to a significant psychological stress whether or not noticeable, but without a defined structural basis. When the symptomatology is related to the nervous system, they are known as conversion disorders (CD). Their etiology has a multicausal and complex nature, having expressions in all the body systems. SD and CD have been scarcely studied in pediatric otolaryngology and are poorly reviewed during medical training. We performed a systematic search on SD and CD in pediatric otolaryngology in PubMed/Medline, SciELO and Cochrane Library databases. We included 49 references, mostly observational studies and narrative reviews. The most described clinical pictures were functional stridor, psychogenic deafness, factitious disorder, and psychogenic vertigo. The diagnostic process requires otolaryngologic and psychiatric evaluations. All studies showed that participants had some relevant affective alteration, although sometimes unnoticeable. Thus, some essential elements are social and school context, family history of SD or CD or any mental disorder. Therapy involves a multidisciplinary approach, including otolaryngologic, audiological, psychological and psychopharmacological interventions. However, evidence supporting specialized interventions is still scarce. Children and adolescents who suffer from SD and CD show clinical and biological findings which are not found in malingering. Proper diagnosis and treatment are related to a good prognosis.

Resumen La otitis media crónica colesteatomatosa (OMCC) complicada es una condición inusual en la época actual. Clásicamente las complicaciones se han clasificado en extracraneales e intracraneales. Estas últimas suponen un alto riesgo para los pacientes debido a la alta morbimortalidad y las secuelas neurológicas asociadas. La atención médica oportuna y derivación a otorrinolaringología junto con las medidas preventivas en edad pediátrica han disminuido la incidencia de los colesteatomas y sus complicaciones. Se presenta a continuación el caso de una paciente de 50 años, atendida en el Hospital Guillermo Grant Benavente de Concepción (Chile) por cuadro de OMCC complicada con meningoencefalitis y vasculitis infecciosa sin consulta precoz por contexto de pandemia COVID-19. Se expone metodología diagnóstica y manejo terapéutico.


Abstract Complicated chronic cholesteatomatous media otitis (CCMO) is an unusual condition nowadays. Complications have traditionally been classified as extracranial and intracranial. The last one entails a higher risk for patients due to high morbi-mortality and neurological consequences associated. Suitable medical care and otorhinolaryngological attention among with preventive measures in pediatric age have decreased the incidence of cholesteatomas and their complications. The following, is the case of a 50-year-old patient treated for CCMO at Guillermo Grant Benavente Hospital in Concepcion (Chile) complicated due to meningoencephalitis and infectious vasculitis without early diagnose due to COVID-19 pandemic. Diagnostic methodology and therapeutic management are exposed.

BACKGROUND: This study aimed to identify a subgroup of recipients at low risk of haemorrhage, bile leakage and ascites following liver transplantation (LT). METHODS: Factors associated with significant postoperative ascites (more than 10 ml/kg on postoperative day 5), bile leakage and haemorrhage after LT were identified using three separate multivariable analyses in patients who had LT in 2010-2019. A model predicting the absence of all three outcomes was created and validated internally using bootstrap procedure. RESULTS: Overall, 944 recipients underwent LT. Rates of ascites, bile leakage and haemorrhage were 34.9, 7.7 and 6.0 per cent respectively. The 90-day mortality rate was 7.0 per cent. Partial liver graft (relative risk (RR) 1.31; P = 0.021), intraoperative ascites (more than 10 ml/kg suctioned after laparotomy) (RR 2.05; P = 0.001), malnutrition (RR 1.27; P = 0.006), portal vein thrombosis (RR 1.56; P = 0.024) and intraoperative blood loss greater than 1000 ml (RR 1.39; P = 0.003) were independently associated with postoperative ascites and/or bile leak and/or haemorrhage, and were introduced in the model. The model was well calibrated and predicted the absence of all three outcomes with an area under the curve of 0.76 (P = 0.001). Of the 944 patients, 218 (23.1 per cent) fulfilled the five criteria of the model, and 9.6 per cent experienced postoperative ascites (RR 0.22; P = 0.001), 1.8 per cent haemorrhage (RR 0.21; P = 0.033), 4.1 per cent bile leak (RR 0.54; P = 0.048), 40.4 per cent severe complications (RR 0.70; P = 0.001) and 1.4 per cent 90-day mortality (RR 0.13; P = 0.004). CONCLUSION: A practical model has been provided to identify patients at low risk of ascites, bile leakage and haemorrhage after LT; these patients could potentially qualify for inclusion in non-abdominal drainage protocols.

INTRODUCCIÓN: La rinosinusitis crónica odontogénica (RSCO) es un proceso inflamatorio/infeccioso de la mucosa nasal y senos paranasales (SPN) de origen dental. Corresponde a una patología subdiagnosticada en la actualidad. El gold standard diagnóstico es la tomografia computarizada. Los tratamientos difieren, desde tratamiento médico aislado hasta médico-quirúrgico de SPN o combinados. OBJETIVO: Caracterizar los resultados clinicos e imagenológicos de pacientes con RSCO en función del tratamiento recibido, para establecer recomendaciones terapéuticas. Describir la frecuencia de subdiagnóstico de esta patología. MATERIAL Y MÉTODO: Estudio descriptivo, retrospectivo. Se incluyeron pacientes con diagnóstico clínico e imagenológico de RSCO entre los años 2013-2017 en un centro de atención médico privado, a los que se realizó una tomografía computarizada cone beam de control. Fueron excluidos aquellos con rinosinusitis crónica, cirugía endoscópica funcional (CEF) previa, cáncer, embarazadas, rechazo a tomografía computarizada cone beam, ausencia de imágenes preoperatorias y menores de 18 años. RESULTADOS: De un total de 27 pacientes, en los operados de CEF (n =24; 89%) la resolución total de los síntomas se logró en 20/24 (83%), resolución parcial 3/24 (12,5%) y persistencia de los síntomas 1/24 (4%). Discusión: De los pacientes con RSCO 22/27 (81%) no tenían reportado el foco dental en el informe radiológico a pesar de ser visible. Aquellos pacientes con sintomatologia postratamiento, hubo factores identificados como la bilateralidad, mala dentadura general, falta de combinación de tratamiento dental y CEF. CONCLUSIONES: La CEF combinada con tratamiento dental concomitante, tiene un alto grado de éxito radiológico y clínico en esta patología. No fue posible demostrar si siempre es requerida la exodoncia con cierre de fístula oro-antral o si se pueden seleccionar pacientes para un tratamiento más conservador.


INTRODUCTION: Chronic odontogenic rhinosinusitis (CORS) is an inflammatory/infectious process of the nasal mucosa and sinuses of dental origin. Actually, it corresponds to an underdiagnosed pathology. The gold standard diagnosis is computed tomography. The treatments differ, from isolated medical treatment to medical-surgical sinuses or combined. AIM: To characterize the clinical and imaging results of patients with CORS according to the treatment received, to establish therapeutic recommendations. In addition to describing the frequency of subdiagnosis of this pathology. MATERIAL AND METHOD: Descriptive, retrospective study. Patients with clinical and imaging diagnosis of CORS between 2013-2017 were included in a private medical care center, to whom a cone beam computed tomography control was performed. Those with chronic rhinosinusitis, previous functional endoscopic sinus surgery (FESS), cancer, pregnant women, rejection of cone beam computed tomography, absence of preoperative images and those under 18 years were excluded. RESULTS: Of all patients operated on FESS (n =24; 89%) the total resolution of the symptoms was 20/24 (83%), partial resolution 3/24 (12.5%) and persistence of the symptoms a 1/24 (4%). Discussion: 22/27 (81%) of patients with CORS had not reported the dental focus in the radiological report despite being visible. Those patients with post-treatment symptoms had identified factors such as bilaterality, general bad teeth, lack of combination of dental treatment and FESS. CONCLUSION: The FESS combined with concomitant dental treatment, has a high degree of radiological and clinical success in this pathology. It was not possible to demonstrate whether exodontia with closure of oroantral fistula is always required or if patients can be selected for a more conservative treatment.
